Estate Planning Resources:
  • The U.S. Living Will Registry
    • If you do not have an advance directive, you should first meet with an attorney to have one prepared, however, if you do have an advance directive and/or medical power of attorney and wish for it to be made available more easily to your health care providers using the U.S. Living Will Registry, complete the Registration Agreement and follow its instructions for submission.
    • This service is provided by Sentara FREE of charge. You can store your advance medical directive on the Registry so it will be available to any health care provider in Virginia and North Carolina as well as any providers across the U.S. Once registered, you will receive an acknowledgment along with a wallet card and stickers for your ID cards that will alert medical professionals that you have a living will on file with the Registry and the 800 number so they can retrieve it. 
    • If you want to have your document registered, you must complete the U.S. Living Will Registry Registration Agreement, giving the Registry permission to store your advance directive and provide it to any healthcare facility that requests a copy.
  • Modifications for Pregnancy:
    • If you have a living will and wish for your living will to be modified in the event you are pregnant, you should speak with an attorney on how to modify your current living will. In the interim, Sentara provides this form, you can use to make special modifications to your advance directive for pregnancy.
